Control Center: GnRH, FSH & LH

Gonadotropin-releasing hormone (GnRH), follicle-stimulating hormone (FSH), and luteinizing hormone (LH) work together to regulate the menstrual cycle, ovulation, and reproductive hormone production. During perimenopause and menopause, this carefully coordinated system changes as the ovaries become less responsive.


These changes affect ovulation and estrogen and progesterone production, helping explain why periods become irregular and many menopause symptoms begin to appear.

Luteinizing Hormone (LH)

How the Hormonal Feedback Loop Changes

During the reproductive years

The hypothalamus releases GnRH, which signals the pituitary gland to release FSH and LH. These hormones act on the ovaries, supporting ovulation and the production of estrogen and progesterone. Ovarian hormones then provide feedback to the brain and pituitary gland, helping regulate the cycle.
 

During perimenopause and menopause

As ovarian follicles decline, the ovaries become less responsive to FSH and LH. Changes in ovarian hormones alter the normal feedback signals to the brain and pituitary gland. FSH rises substantially, LH also increases, and ovulation eventually stops.

Why GnRH, FSH and LH Matter During Menopause

Together, GnRH, FSH, and LH form an important communication pathway between the brain, pituitary gland, and ovaries. Changes in this system reflect the transition from regular ovulation to menopause.

For many women over 45 with typical menopause symptoms, routine hormone testing is not usually necessary because hormone levels can fluctuate considerably during perimenopause.

  • Menopause symptoms occur before age 45

 

  • Periods stop unexpectedly at a younger age 


  • Premature ovarian insufficiency is suspected 


  • Menstrual changes have an unclear cause 


  • Fertility or ovulation is being evaluated 


  • Another hormonal or medical condition needs to be ruled out

Treatment & Support Options

Menopause treatment generally focuses on managing symptoms rather than directly changing GnRH, FSH, or LH levels. 

Medical:

  • Menopausal hormone therapy (MHT) may help manage symptoms such as hot flashes and night sweats when appropriate. MHT also affects the hormonal feedback system and can lower elevated FSH and LH levels, although this is not usually the primary goal of treatment. 


  • Nonhormonal treatments are available for certain menopause symptoms when hormone therapy is not appropriate or preferred. 


  • GnRH medications affect the signaling between the brain, pituitary gland, and ovaries. They are used for certain medical conditions, such as endometriosis or uterine fibroids, rather than as routine treatment for natural menopause. 


Treatment should be individualized based on symptoms, medical history, risks, and personal preferences.
